    SLEEVE GASTRECTOMY

    Today, the most performed obesity surgery in the world is sleeve gastrectomy. This is because it is both very effective and relatively easy to perform. Like other obesity surgeries, it is done laparoscopically (by making 5 incisions in the abdomen and using special long instruments and a camera).
    PERFORMING THE SLEEVE GASTRECTOMY
    In this surgery, the blood vessels along the greater curvature of the stomach are first cut to mobilize the stomach. Then, for calibration, a tube or bougie (36F) of a certain thickness is inserted through the mouth and passed through the pylorus at the stomach’s exit. Next, starting 2-3 cm away from the pylorus, the stomach is divided from bottom to top using a stapler (a special device that simultaneously places three rows of staples and cuts). About 80% of the stomach is removed. The staple line is either left as is or reinforced with stitches, adhesive, etc. We prefer to use stitches. This creates a gastric tube with an average volume of 70-110 cc. Tests are performed with methylene blue and air. One drain is placed for drainage and control purposes.
    THE WEIGHT LOSS EFFECT OF THE SLEEVE GASTRECTOMY
    Sleeve gastrectomy causes weight loss in two ways:
    1- Hormonal effect: Hormones responsible for appetite, especially Ghrelin, are secreted from the stomach’s fundus. Because this part is removed in sleeve gastrectomy, patients’ appetite decreases, leading to weight loss.
    2- Restrictive effect: In sleeve gastrectomy, the stomach volume is reduced, so patients eat less and feel full more quickly.
    POST-SLEEVE GASTRECTOMY PERIOD
    After sleeve gastrectomy, a special diet is followed for the first month. Sequentially, a liquid, puree, and soft diet are followed. During this period, dietitian supervision is essential. Solid foods are introduced at the end of the first month. Water intake starts the day after surgery and gradually increases. Protein support is mandatory for the first month. Blood-thinning injections started the day before surgery are continued for at least 10 days. Compression stockings are worn during the surgery and are recommended for at least 10 days. Walking is recommended as exercise in the first month, with duration and pace gradually increasing. Aerobics, fitness, and swimming can be started after one and a half months.
